*where a serious case review has been evaluated as ‘inadequate’ the LSCB should convene an SCR Panel, to be chaired by an independent person, to reconsider the review. The LSCB is then required to submit to Ofsted within 3 months, an action plan that addresses the inadequacies of the review.
