The Panel will prepare an annual report for the Local Safeguarding Children Board. The Report should contain:
- the total number of deaths reviewed;
- recommendations made by the Panel about actions required to prevent child deaths in the future;
- relevant information about the child deaths reviewed ; and
- review of actions taken to implement recommendations from the previous year’s report
Care will be taken to ensure that no information is included in the annual report which may lead to the identification of individual children or families.
The Local Safeguarding Children Board will use the information gathered to:
- disseminate the lessons to be learnt to all relevant organisations;
- ensure that relevant findings inform strategic and policy-making forums in the area
- improve policy, professional practice and inter-agency working to safeguard and promote the welfare of children; and
- ensure that data relating to child deaths is submitted to relevant regional and national initiatives to identify lessons on the prevention of unexpected child deaths.
Information is collated nationally to provide information on modifiable factors that can be actioned to prevent future deaths.
