The full Serious Case Review Report consists of the:
- Individual management reports and any independent professional reports
- Overview Report
- Executive Summary
- Action Plan
The Serious Case Review Report must be quality assured by the Serious Case Review Panel and then presented to the Chair of the Local Safeguarding Children Board. The Serious Case Review Panel has then discharged all its duties, and follow-up tasks become the responsibility of the Serious Case Review Sub Group on behalf of the LSCB. These include
- Disseminating the overview report/executive summary and key findings to relevant interested parties
- Publishing the overview report and executive summary
- Implementing actions for which the LSCB is responsible
- Monitoring timely implementation of the SCR Action Plan
- Amending the action plan and/or the SCR Report as necessary on receipt of the evaluation letter from Ofsted
- Making arrangements to provide feedback and debriefing to staff
- Managing any media interest
The LSCB must approve the Serious Case Review Report and provide an anonymised copy to Ofsted, the relevant Health Authority, the Department of Education and any other relevant inspection body
If the Serious Case Review is evaluated by Ofsted as inadequate, a Serious Case Review Panel, chaired by an independent person, should be convened to reconsider the review. An action plan must be submitted to Ofsted that addresses the inadequacies of the SCR within 3 months
The Review process is concluded when the action plan has been implemented, the LSCB has made a formal decision that the Serious Case Review is concluded and Ofsted has been informed of this decision.
