What are the Timescales in Serious Case Reviews?

The Chair of the Local Safeguarding Children Board must make the decision about whether to initiate a serious case review within one month of the date of receiving notification of the incident from the LSCB Business Manager.

Reviews should be completed within six months of the decision that they be undertaken, unless it becomes clear – once the review is in progress – that six months will not be long enough (e.g. ongoing criminal proceedings) in which case the LSCB must inform Ofsted of the new completion date, the reason for the extension and provide details of:

  • Recommendations for action which are not dependent on the Serious Case Review being completed
  • Actions taken to date
  • Reason for extension to the timescale
  • Revised completion date

If a Serious Case Review is judged inadequate by Ofsted, an action plan must be submitted to Ofsted that addresses the inadequacies of the SCR within 3 months.
