Each organisation identified and informed of their involvement in the Review by the Chair of the Serious Case Review Panel should immediately:
- Identify a senior manager and an IMR Report Writer
- Secure, and record the date when secured, all their records relating to the child and family to guard against loss or interference
- Commence the individual management review following the North West Individual Management Reviews pro forma. If there are specific issues in individual cases not covered by the format, these should be added to the review.
- Draw up a chronology of their involvement with the child and family
If a case is known to give rise to concerns within the individual organisation, the individual management review may commence before notification is received in order to learn early lessons and to address any urgent operational or management issues.
The designated safeguarding health professionals completing the individual management review should review and evaluate the practice of all involved health professionals and providers and produce an integrated health chronology and a health overview report focusing on how health organisations have interacted together.
If a child has died in custody, the Serious Case Review terms of reference should set out how the information from the subsequent fatal incidents investigation should inform the Serious Case Review.
The Review must be completed by:
- Interviewing staff and others as necessary. A written record of any interviews must be made and this record must be shared with the interviewee.
- Reading all the secured case records
- Provision of feedback and debriefing for involved staff and others on completion of the review report
The senior manager in the individual organisation should agree the findings of the individual management review report and accept responsibility for ensuring that any recommendations in the report are acted on before sending the report and the chronology to the chair of the Serious Case Review Panel.
