The referrer should keep a written record of:
- Discussions with the child
- Discussions with the parent
- Discussions with managers
- Information provided to the Social Worker
- Decisions taken (clearly timed, dated and signed)
The referrer should confirm verbal and telephone referrals in writing, within 48 hours, using the multi agency referral form.
